ARKANSAS HUMAN DEVELOPMENT CORPORATION:

The Arkansas Human Development Corporation developed the Promotoras de Salud Project (Health Promoters program) in 1998. The Promotoras de Salud program currently has five Health Promoters that provide bilingual health education and prevention information to members of the Hispanic and Latin-American communities in Center Arkansas.

ARKANSAS BIRTHING PROJECT:

The Arkansas Birthing Project’s Sister-Friend Program sends women to the home of pregnant women at risk for poor health outcomes. The Sister-Friends will visit the pregnant women’s home and offer personal and professional support. The Sister-Friends work with the pregnant women up to the first year of life of the baby.

BOSTON MOUNTAIN RURAL HEALTH CENTER, INC.:

Boston Mountain Rural Health Center, Inc. provides comprehensive primary care services to patients. Their mission is to “to provide high quality, accessible, affordable, patient-centered health care to improve the well-being of our patients in the communities we serve.”

COMMUNITY CLINIC OF NORTHWEST ARKANSAS:

Community Clinic of Northwest Arkansas has been providing care to Northwest Arkansas for around 40 years. Their goal is to provide services to the underserved and increase access to comprehensive quality care.

HARMONY:

Harmony Health Clinic is a non-profit clinic that “seeks to understand and serve the health and wellness needs of the medically uninsured and underserved” by providing access to quality medical care at no cost. Harmony Clinic has been serving the community for about eight years.

MARSHALLESE EDUCATIONAL INITIATIVE (MEI):

MEI is a not-for-profit organization that “promotes the cultural, intellectual, and historical awareness of the Marshallese people and facilitates intercultural dialogue to foster positive social change.”

MERCY HOSPITAL DIABETES PREVENTION PROGRAM:

The Diabetes Prevention Program and Mercy Hospital is “designed to reduce the instance of type 2 diabetes in the nation.” This program is led by trained lifestyle coaches that work to keep the patients motivated and on track for their healthy lifestyle goals.

OUTREACH CLINIC IN NWA- JOSEPH BATES WASHINGTON COUNTY SPRINGDALE:

The Joseph Bates Outreach Clinic located in Springdale, is an Arkansas Department of Health local health unit. This health unit offers community services, family planning and WIC, Immunizations, HIV/STD testis, and a TB prevention program.

ST. BERNARDS POPULATION HEALTH:

St. Bernards utilizes individuals from multiple professions and disciplines in working together to meet the healthcare needs of the community. The goal of population health is to help people receive care so that they can stay out of the hospital.

TRI COUNTY RURAL HEALTH NETWORK:

Tri-County Rural Health Network's (TRHN) mission is to improve access to health care. THRN's primary initiative is the Community Connector Program (CCP). CCP deploys trusted community members called "community connectors" to link those needing long-term care to home and community-based services by addressing socio-cultural barriers and system navigation.

UAMS EMERGENCY DEPARTMENT (ED):

The UAMS Arkansas Center for Health Disparities Community Engagement Core is partnering with the UAMS Department of Emergency Medicine to employ a CHW through the hospital Emergency Department. The CHW is a member of the health care team and works one-on-one with patients who frequently visit the Emergency Department. With the goal of reducing medically unnecessary Emergency Department visits, the CHW provides patients with information about and navigation assistance to primary and specialty care services as well as social services and community-based resource.

UAMS FMC LITTLE ROCK:

The UAMS Family Medical Center in Little Rock is a Level 3 Patient Centered Medical Home that offers comprehensive care for the entire family. They offer stages of care from prevention measures and patient education, primary care, treatment of injuries, management of chronic health concerns, and outpatient procedures.

UAMS NORTHWEST:

The UAMS Northwest Regional Campus is an extension of UAMS’s medical education, research, and clinical mission in northwest Arkansas. They have two family medicine clinics, located in Fayetteville and Springdale, and many other types of clinics spread around the Northwest region. UAMS Northwest has many partners in the area as well.

UAMS NORTHWEST OFFICE OF COMMUNITY HEALTH AND RESEARCH (OCHAR) AND NORTH STREET CLINIC:

The North Street Clinic employs CHWs in an effort to provide culturally appropriate care to Marshallese patients in Northwest Arkansas. In conjunction, the nurse manager of the clinic does outreach to local clinics and hospitals to promote the utilization of Marshallese and Hispanic CHWs as a part of the clinical care team. OCHAR recruits CHW trainees from Marshallese and Hispanic churches, as well as directly from within health care organizations, with a goal of maintaining a running list of trained CHWs to share with area health care providers and organizations wishing to utilize CHWs.

VETERANS ASSOCIATION:

The Veterans Association implements a Peer Support Specialist program for Veterans that need assist in mental health and substance abuse treatment. A Certified Peer Support Specialists is a Veteran in recovery from mental illness or substance use disorders that provides support to Veterans currently seeking help.
